Welcome and thank you
for allowing us to provide you with your family medical care.
We would like to take this opportunity to familiarize you with our
office.
Dr. Cook is a Board
Certified Family Practice physician who has moved from the Daytona Beach area
where she practiced for the last 3 years. She
completed her residency in Family Practice in Ormond Beach, Florida and received
her Doctor of Osteopathy degree from the West Virginia School of Osteopathic
Medicine.
Office Hours
Our office is open
from Monday through Friday, 9 am to 5 pm.
We are available for patients ages 5 and older. We ask that a parent or guardian accompany all patients under 18 years of age to all visits.
Appointments are
available so that the doctor will have ample time to address your concerns.
Walk-ins are welcome and will be worked into the schedule.
We do ask that if you are unable to keep your appointment, please kindly
give 24 hours notice, otherwise we reserve the right to charge for time
reserved.
Medications
Please bring with you
all medications that you are currently taking so that we will have an idea of
your medications, their dosages and your refill needs.
For your convenience,
our office will file insurance. However,
co-payments and deductibles are due at the time of service.
We accept cash, checks and credit cards.
All co-payments are
payable when you check in at the front desk.
We do accept most
insurances. Please inform our
receptionist of your insurance information and provide your insurance card at
the time of your visit so that we may file your insurance appropriately.
Please also keep us updated with any changes to your name, address, or
insurance information.
After hours and on
weekends and holidays, Dr. Cook is available by calling the office at
863-701-7188. In case of an
emergency, please call 911 for immediate assistance and transport to your
nearest emergency room.
